A Malaysian clinic clears its ads with the Medicine Advertisements Board before they run, so the creative is finished long before the budget is. That makes the job a small set of assets built exactly right, with space for the approval number designed into the artwork while the layout is still soft. Time is the inventory, and it does not keep
A clinic sells time. Tuesday's 3pm slot exists once, and if nobody books it that revenue is gone, with no amount of spend next month bringing it back. A product brand can live with a slow-burn awareness campaign. A practice with two treatment rooms needs enquiries this week, from the postcodes it can actually serve. The catchment is smaller than most owners like to admit. Patients choose on drive time and parking as much as on reputation, so a beautifully argued ad shown 30 kilometres away buys nothing at all. The field inside that circle is crowded. The Ministry of Health counted 11,067 private medical clinics and 4,309 private dental clinics as at 31 December 2024, alongside 21,013 doctors working in the private sector (Health Facts 2025). A patient scrolling past your ad in a Klang Valley suburb has a dozen alternatives within fifteen minutes of home. The money behind that density is real money. The Department of Statistics Malaysia's Economic Census 2023 recorded 19,847 private health and social work establishments generating RM28.0 billion in gross output and employing 156,193 people, for reference year 2022. The Malaysia National Health Accounts put private-sector expenditure on health at RM37.7 billion in the same year. Patients feel the cost of that growth. Bank Negara Malaysia's Financial Stability Review for the second half of 2024 recorded medical payouts by life insurers and family takaful operators of RM6.2 billion in that half, against RM5.3 billion in the first, and attributes the rise to higher average treatment costs and higher utilisation. That is an insurer and takaful figure, not a national health bill, and it still tells you something useful about the person reading your ad: they are weighing a real cost. So choose the number you report to your partners with care. Booked consultations per RM spent, by branch, with a show rate beside it. Reach flatters. An enquiry that never turns up leaves the chair as empty as no enquiry at all.
Approval sits in front of the launch
Most advertisers write, launch and edit inside one afternoon. A Malaysian clinic works the other way round, and the sequence is set by statute. The law splits three ways, and the split matters. Section 3 of the Medicines (Advertisement and Sale) Act 1956 (Act 290) bars advertising to the public altogether for the twenty conditions in the Act's Schedule, so for those there is nothing to apply for. Section 4B requires the Medicine Advertisements Board's approval before an advertisement presents an article as a medicine, an appliance or a remedy for treating or preventing a condition. Section 4A carries the same requirement to advertising a skill or a service for treatment, which is the route a practice usually travels. A first conviction under section 5(1) carries a fine of up to RM3,000, up to a year's imprisonment, or both, on the Attorney General's Chambers reprint of Act 290 as at 1 May 2013. The Board's approval reference is the KKLIU number, and here honesty is worth more than confidence. Section 4B says nothing about displaying it. The display requirement sits in the Board's own guideline, and the copy of that guideline we were able to read came from a third-party mirror, not the Ministry host. Treat display on the published advertisement as the working assumption, design the space for it, and confirm the current procedure with the Board before you build a layout around it. Registration is a separate permission: a registration number says the product may be sold, and an approved medicine advertisement generally has to carry that number alongside the KKLIU one. Then the production consequence, which is the part that reshapes a clinic's week. Approval attaches to the specific creative submitted. Reword a headline, recut a video or swap a hero image and the clearance may no longer cover the asset, so "the same ad with a sharper headline" is a question for the Board, not a five-minute edit before lunch. An approval reference that has to sit legibly on the image or in the video frame is also a layout problem. It has to survive the square crop, the 4:5 feed crop and the 9:16 Story, and it cannot be the element your headline sits on top of. Dentistry answers to its own rulebook. The Malaysian Dental Council's Code of Professional Conduct and its Guidelines and Provisions for Public Information, issued under the Dental Act 2018 and updated in July 2022, draw the line at soliciting, which is a different discipline from claim control. Meta then applies a gate of its own. Its Health and Wellness standard, whose live page carries a single changelog entry dated 26 December 2024, requires ads for dietary, health or weight-loss products and for cosmetic products, procedures and surgeries to be targeted to people aged 18 and over, and prohibits negative self-perception framing. A plain consultation-booking ad for a GP practice is less clear-cut on that age gate, and 18-plus is a sensible default in a category whose patients are adults anyway. The detail belongs where it already lives. Read them before you brief anything, and take anything borderline to the Board or your regulatory adviser.
Creative is inventory, so build it in batches
Once approval attaches to a creative, the useful unit of work stops being an ad and becomes a batch. Everything you want in market next quarter should exist before you submit, each piece already cut to the placements you intend to buy. Improvising in week six is the expensive path, because the fastest route back to market is a creative that is already cleared. Start by reading the category. Thirty Malaysian clinic creatives in our archive show one dominant habit: the hook names the device or the protocol, which is the thing the clinic itself is proudest of. The second most common shape puts the practitioner on camera, introducing themselves and then explaining something genuinely hard, such as the worst ninety days after a hair transplant. Video carries fourteen of those thirty, static twelve, and most of the video is cut 9:16. The language mix is close to even between English and Chinese with a few in Malay, which matters because clearance is granted per version and a three-language rollout is three submissions. Two absences are as loud as the hooks: only one of the thirty carries an approval reference on the creative, and none carries a results-may-vary disclaimer (AdPlay.ai archive, 2026). Those are representative angles from real advertisers, never a claim that a particular ad is running today. One honest limit sits on all of it: the archive stores creative and its metadata, never anyone's spend, so read it for angles and formats and never for a competitor's budget. One more absence is worth a decision. No example in that set opens on a consultation or an assessment. Either the category has tested that opening and moved on, or it is sitting there unclaimed, and a batch is cheap enough to find out. So plan four pieces: the credential-led one, a procedure explainer, an aftercare piece, and the consultation opener if you decide the gap is worth taking. Then produce. On a dropped pin the radius runs from 1 km to about 80 km in markets that display metric units (a 1-mile minimum to a 50-mile maximum where they do not), and a fresh pin starts at the tightest setting, so widening it is a deliberate act. A named city behaves differently: Ads Manager will not let you shrink the circle below the city's own footprint, which is why the pin is the tool for a single branch. Choose a postcode or a whole state and the radius option disappears, because those target the named area as it stands. One thing you cannot control is who inside the circle counts as local. Until 1 May 2023 Meta offered a presence dropdown and a clinic could buy residents on their own. Meta removed it without an announcement. Every location set now runs on one combined setting, people living in or recently in the location, so a practice unavoidably pays to reach some people who were merely passing through. The levers that remain are a tighter circle, exclusions where the catchment is an odd shape, and creative that self-selects by naming the neighbourhood. Most enquiries here arrive as a chat. A click-to-WhatsApp campaign drops the patient into a conversation with a prefilled opener, which matches how Malaysians ask "is Saturday free?" and "how much is the consultation?" before committing to anything. That messaging destination is configured on the Meta side, in Ads Manager, and what you bring to it is the creative and the offer. Lead forms work too. Either way the booking is won in the reply. An enquiry is warm for minutes, not hours, and a message answered with two concrete slots does more than one answered next morning with a price list. Treat first-reply speed as an operational target with a name against it, because it belongs to the rota and no bid adjustment substitutes for it. Measurement needs its own plan, and the constraint is applied account by account. Search Engine Land documented Meta's health and wellness advertising restrictions in March 2025: a three-tier ladder surfaced in Events Manager, under the Settings and Diagnostics tabs, running from limits on core setup, to a restriction on standard events such as Add to Cart and Purchase, to full optimisation restrictions that stop an account optimising toward mid-funnel and lower-funnel events while upper-funnel campaigns keep running. Accounts get moved through those tiers, so this is a status to check, not a blanket rule on every health advertiser, and none of it shows in Account Quality. If your account is tiered, optimise toward the signals still available to you, such as landing-page views, video views, message opens or lead submissions, and do the real accounting in your own systems. Tag every enquiry with the creative and the branch that produced it, mark it when the patient shows up, and read cost per booked consultation by branch at month end. A quarter in the life of a clinic marketer
Week one, research and shortlist. Spend an hour in the archive on how practices in your niche open, which treatments they lead with, and how long each ad has been live, then write down four angles worth owning and decide which branches each one runs in. Resist the fifth angle. A batch you can actually produce beats a plan you abandon. Week two, produce the whole batch. Generate images and copy from the brand kit, cut the doctor explainer with captions, resize every concept for feed, Stories and Reels, and build the approval-reference field and disclaimer space into the layout while the layout is still soft. Have your regulatory adviser read the copy before it goes near the Board. Week three, submit and wait. Build nothing new while approval is pending. Use the time on the parts nobody schedules: WhatsApp reply templates, who answers enquiries in business hours and who covers Saturday, and a booking system that records where every enquiry came from. Once cleared, launch small. One branch, two or three approved creatives, a tight radius, a budget you would not mind losing. Then leave it alone for a few days, because early numbers wobble before they settle. Read cost per enquiry by creative and cost per booked consultation by branch, and move budget toward the ads that fill chairs. The rest of the quarter is repetition without the production panic. Rotate the approved set, widen the radius around the branch that fills up first, and start the next batch's research in the final month so submission lead time never lands on the critical path. File the approved artwork, the submission and the approval reference together, because a year from now the only person who remembers which version was cleared is the one who wrote it down. The practices that advertise well here are the ones whose cleared creative never runs out.

No, the primary rulebook is different. Dental publicity sits under the Malaysian Dental Council's Code of Professional Conduct and its Guidelines and Provisions for Public Information, issued under the Dental Act 2018 and updated in July 2022, which require publicity to be legal, decent and truthful with high regard for professional propriety, and which treat soliciting as the central offence. One nuance is widely misread: the 2022 update permits displaying tariffs so patients can compare treatment, while wrapping the same price in discount or limited-time framing slides straight back into soliciting. How do I show clinic ads only to people near my branch?
You cannot buy residents on their own, and that is the first thing to know. Meta merged its four presence options into one on 1 May 2023, so every location set now reaches people living in or recently in the area. What you control is the shape. Drop a pin on the outlet and set a radius: 1 km to about 80 km in metric markets, 1 mile to 50 miles where units are imperial, starting at the tightest setting. A postcode or a state removes the radius option and takes the whole area as it stands. Run one campaign per branch so each has its own pin, budget and read, start tighter than feels comfortable, and widen only once a branch is filling.
Can clinic ads use before-and-after photos in Malaysia?
Sometimes, and it turns on the treatment and on which rulebook you are reading. The Malaysian guidelines say before-and-after visuals must not exaggerate or mislead about changes in the body, and patient testimonials come with consent and disclaimer expectations. Meta's Health and Wellness standard adds its own limits: side-by-side comparisons are banned for weight loss, with an exception for fitness classes, and banned for wrinkle and anti-aging treatments such as Botox and dermal fillers, while general cosmetic products, procedures and surgeries are expressly allowed to show a transformation when the audience is aged 18 and over and the creative avoids negative self-perception tactics. What do Facebook ads cost for a clinic in Malaysia?
